Patient Rights
As a patient, you have the following rights:
1) The right to be treated with respect, consideration, and dignity.
2) The right to be provided the appropriate privacy in exam rooms and waiting areas.
3) The right to be treated with respect regarding your psychological, cultural, and spiritual needs.
4) The right to communicate with your healthcare professional and staff regardless of your primary language or impairment of hearing or vision. Reasonable attempts will be made to accommodate your communication needs.
5) The right to be provided current information concerning your diagnosis, treatment, and prognosis.
6) The right to participate in decisions involving your health care, except when such participation has the potential to affect your medical safety.
7) The right to be provided high quality care delivered in a safe, timely, efficient, and cost-effective manner.
8) The right to receive considerate and respectful care in a clean, nonsmoking, and safe environment.
9) The right to receive care in a safe and secure setting without discrimination of race, color, religion, sex, national origin, disability, sexual orientation, source of payment, or age.
10) The right to examine and receive a full explanation of any medical charges.
11) On completion of the treatment, the right to request a copy of a medical summary report.
12) The right to receive information necessary to give informed consent prior to the start of treatment. This information shall include the specific procedure and/or treatment, the reasons for it, the possible risks and the benefits of the procedure or treatment.
13) The right to register complaints and recommend changes in policies and services to the facility’s staff and to have the facility respond to you, if requested.
14) The right to be informed of the credentials of healthcare professionals involved in your care.
15) The right to be informed of provisions for after-hour care and emergency care.
Patient Responsibilities
1) Provide, to the best of your ability, complete and accurate information about your present medical problems, past medical history, hospitalization, medications, allergies, and any other pertinent information necessary to properly diagnose and treat you as our patient.
2) Follow the treatment plan prescribed by your facility and participate in your care.
3) Inform your facility if you do not intend to follow your treatment plan.
4) Pay the required medical costs as per the notified payments schedule.
5)Inform the facility of any changes in your address and telephone numbers without delay.
6) Ask questions when you do not understand what you have been told about your treatment by doctors or what you are expected to do.
7) Notify the facility or Clinic as soon as possible if you are going to be late for your advised appointment.
8) Be respectful to all health care professionals and staff, as well as other patients.
9) Be respectful of the office environment and the property of others.
10) Be responsible for all personal belongings left unattended in the facility.
11) Respect the rights of others and abide with the code of conduct (under).
